By clicking on the Submit button, I understand that the submitted file must be a copy of the payment plan offered to the owner of the Subject Property by the association and/or Alterra. I understand that if the submitted file is not signed by the owner, is incomplete, or any efforts have been made to modify its terms, the payment plan will not become effective, and will result in further collections fees and costs added to the delinquent account. The payment plan will not become effective until a copy of the payment plan signed by the association and/or Alterra is forwarded to me by mail at the mailing address provided in this form. I further understand and acknowledge that I have carefully reviewed the payment plan and have consulted my own attorney, and that I understand my legal obligations set forth therein.
